PASSPORT APPOINTMENTS

The Town Clerk’s Office is a Certified Passport Acceptance Facility with qualified clerks who can help you with all of your passport needs.  When you have your passport application processed here, you not only receive the finest service, you are also helping to keep Town property taxes among the lowest in Monroe County!  All passport processing fees collected in Henrietta stay in Henrietta, which helps to support the many fine programs and services offered to Henrietta residents and the greater community.

Please schedule an appointment by calling our office at (585) 359-7035.  At that time, we will go over any questions you may have and be sure that you have all the documentation needed to apply for a U.S. Passport.  You do not have to be a Henrietta resident for Passport services.

Passport Appointments are available on Tuesdays between 9:00 a.m. and 12:30 p.m. and Thursdays between 12:00 p.m. and 4:30 p.m. Please call us at (585) 359-7035 or email us at townclerk@henrietta.org to set up an appointment.  
 

WHAT YOU NEED TO BRING WITH YOU
The following list will guide you on what each person will need to bring with them.  Please note that ALL passport applicants must appear in person.
 

  • Completed Passport Application:
    You can pick up Passport Applications in the Town Clerk’s Office Monday – Friday from 9:00 a.m. – 5:00 p.m.  Passport Application forms are also available on the U.S. Department of State’s website at the link below.  Please note that if accessing the form online, you will need to print it out single sided.  Please do NOT sign the application until you are issued the Oath by one of our Acceptance Agents at your appointment.

  • ​Proof of U.S. Citizenship: (ONE OF THE FOLLOWING)

    • Birth Certificate with a Raised Seal
      The Birth Certificate must also include a filing date, filed within one year of birth date and a County Registrar’s signature. All birth certificates submitted for both adult and minor passport applications MUST contain the names of the parents.
       
    • Naturalization Record 
       
    • Previous Passport
      If you have had a previous passport, it MUST be included with your Application.

 

  • Proof of Identity:
    Valid Driver’s License or Official Photo I.D. card.  If your license is from out of state, please furnish an additional form of I.D., including but not limited to school I.D., state-issued I.D., work I.D., or expired driver’s license.
     

  • Recent Color Photograph:
    You can get your passport photo taken here at the Town Clerk’s Office for $10.00 per applicant.  If you bring a passport photo to the appointment, it must have been taken within the last six (6) months to be considered valid and meet the U.S. Department of State’s photo requirements.
     

  • Check or Money Order:
    Please bring at least one (1) check or money order to be mailed along with your application for the Execution Fee and Optional Expedite Fee, made payable to the U.S. Department of State.  Do not include Execution or Photo Fees with the Application Fee!

The Execution and Photo Fee(s) are payable to the Town of Henrietta.  We are able to accept cash, check, Visa, MasterCard, Discover, and American Express cards for payment. Please note that there is 2.45% service fee or a minimum of $1.55 service fee for credit and debit card payments.  This service fee is charged by the payment processor, not by the Town of Henrietta.
 

  • Additional Requirements for Minor Passports (Under Age 16)

  1. The minor must be present at the appointment; and

  2. Both parents must be present, along with identification for each, such as a valid Driver’s License or Passport.  If any parent’s license is from out of state, please furnish an additional form of I.D., including but not limited to school I.D., state-issued I.D., work I.D., or expired driver’s license.

PASSPORT RENEWALS

**PLEASE NOTE THAT WE ARE NOT ABLE TO PROCESS RENEWAL APPLICATIONS IN OUR OFFICE - WE CAN ONLY TAKE THE UPDATED PHOTO**

Passports can be renewed BY MAIL if:

  • Your most recently issued passport book and/or card was issued within the last 15 years and if it had a normal 10 year validity;
  • ​You can submit your most recently issued passport book and/or card if it has not been lost, stolen, mutilated, or damaged;
  • You were at least 16 years old when your most recently issued passport book and/or card was issued;
  • Your name is the same as what’s shown on your most recently issued passport and/or card OR you are able to submit proper documentation to reflect a name change.

Renewal Passport Photo

One of the requirements in a renewal passport application is an updated photo taken within the last 6 months. Passport Photos for renewal applications are available in the Town Clerk's Office. However, at this time we are only able to offer printed photos and therefore cannot provide digital photos for online passport renewals.

The fee for a passport photo is $10.00 per person. We are able to accept cash, check, Visa, MasterCard, Discover, and American Express cards for payment.
